There are lots of possible causes, some are okay like airdrops and some are fucked up like when a token is launched and a great deal of its supply is snipped and then sent to different wallets in order to disguise a rug.

For example, imagine a coin launchs with 1B supply and a sniper buys 800M then sends to different wallets. Although 80% of the supply is being held by one person/team, it won't show on top holders because it was spread to many wallets. Then they can start selling from those wallets (and that's one of the reasons you see a sell without any buying) or they can simply sell it all at the same time. Both cases reflect a rug pull, the difference is only that the first case is a slow rug
