Had this problem with 4o today. It generated a working ASCII art maze solver in Python and then gave pretend output when I asked it to run the code. Asked it again and it did the same thing. Switched the session to use gpt4 and it said it can't run code. Pasted the code in a fresh gpt4 session and it ran it. Tried from scratch in a fresh 4o session and it worked.

It's really annoying how sometimes when it does something wrong it will reliably stay wrong until you kill the session and start over.
